
Having a loved one referred to hospice brings with it a flood of emotion as well as many questions. What does Hospice mean? Is this the end? What do we do now? Does this mean my loved one must be moved? What is available through Hospice?
We understand...many of us at Bethany have experienced these emotions first hand. That’s why we want to provide you with as much support and answers to questions as possible.

What to Expect Upon Referral

Physician’s Order: Bethany Hospice services are initiated by a hospice order from a physician. Simply ask your doctor for an order for a hospice evaluation or, if you wish, we can make that request for you.

Evaluation: Next, a member of the Bethany Team will visit the patient and complete a hospice evaluation. This may include reviewing current medications, discussion of activities of daily living, and review of the patient’s medical history.

Informed Consent: If a patient is determined to be medically appropriate for hospice services, we will contact you to schedule a meeting to discuss hospice in detail. We offer a very flexible schedule to accommodate the needs of family, including evening and weekend appointments.

Nursing Assessment: Upon completion of consents, a Registered Nurse will visit the patient and complete a comprehensive nursing assessment. The nursing assessment is used to develop a plan of care and helps us determine the initial visit frequencies. The physician is notified that the admission is complete and he or she provides us with any appropriate orders for care. The RN will tell you what day you can expect your next visit and how many times per week we will have the patient on our schedule.

Hospice Plan of Care: The entire hospice team, including our on-call staff, is notified of all new admissions. We welcome the input of our patients and families. Feel free to call us at any time whether you have a question, a medical issue, or would just like a friendly ear to lend you some support. We are members of your team and we value your contributions.
