This office position is available for the multi-tasking LPN. The position would encompass job descriptions for both the Quality Assurance Coordinator and Hospice LPN for skilled professional nursing care to patient/family/caregiver as directed by the supervising Registered Nurse or the Director of Clinical Services. 

 

TITLE: Quality Assurance Coordinator

SUPERVISOR: Director of Clinical Services

RESPONSIBILITIES

 

1.      Establish and maintain ongoing efforts to educate the hospice team to improve documentation.

2.      Provide regularly scheduled chart audits in compliance with the timeline established by company policies and procedures and at other times as warranted.

3.      Compile written reports of findings and results of audits and present monthly to the DCS and quarterly to the QAPI committee.

4.      Organize and schedule all pertinent staff to assist with audits and ensure timely completion of the assignments given.

5.      Provide teaching and training to staff to assist in ongoing professional development of the team.

6.      Identify potential areas for quality improvement and communicate to company management.

7.      Participate in staff meetings, department meetings, team meetings, briefings, in-services, committees and other related activities as needed.

8.      Maintain compliance with local, state and federal regulations and company established personnel policies and procedures.

9.      Meet or exceed delivery of company service standards in a consistent manner.

10.  Conduct all business activities in a professional and ethical manner.

 

JOB SUMMARY

 

Conduct and participate in quality assurance review and improvement programs.  Provide written and oral reports of quality assurance findings. Provide training and education to hospice team as directed by the Director of Clinical Services.  Represent the company in a competent, professional and responsive manner.  Interact with the patient care team consistent with team-building and professional culture established by the company.  Maintain high quality customer service standards in compliance with federal and state regulations and guidelines.  Manage related expenditures in a fiscally responsible manner in accordance with the budget.  Travel as necessary.

 

TITLE: LICENSED PRACTICAL NURSE

SUPERVISOR: DIRECTOR OF CLINICAL SERVICES

 

RESPONSIBILITIES

 

1.      Ensure quality and safe delivery of Hospice services to patients, families and caregivers.

2.      Implement nursing plan of care as directed by the interdisciplinary team and Director of Clinical Services.

3.      Provide professional nursing care by utilizing all elements of nursing process as directed by the plan of care.

4.      Provide accurate and timely documentation of patient services provided.

5.      Provide patient/family/caregiver education pertinent to the diagnosis and plan of care.

6.      Participate in the coordination of hospice services, appropriately report the need for interdisciplinary team members (e.g. HHA, OT, PT, ST, MSW, SC, Dietician) to the Director of Clinical Services.

7.      Use equipment and supplies effectively and efficiently.

8.      Provide appropriate pain and symptom management.  Evaluate patient’s response to treatments and medications.

9.      Complete and maintain all patient records at patient’s residence, submit accurate and relevant clinical notes regarding patient condition and care.

10.  Observe patient for changes in condition and report changes to appropriate supervisory personnel.  Record changes and outcomes as appropriate.

11.  Assist patient with activities of daily living.  Facilitate patient effort toward self-sufficiency and comfort.

12.  Teach patient/family/caregiver self-care techniques as appropriate, working with other interdisciplinary team members.

13.  Provide and maintain a safe environment for the patient.

14.  Fulfill productivity requirements on an average daily census basis as determined by acuity level.

15.  Assist patient/family/caregiver and other team members in providing continuity of care.

16.  Work in cooperation with patient/family/caregiver and interdisciplinary team members to meet the emotional needs of the patient/family/caregiver.

17.  Perform within the limits of preparation and experience.

18.  Manage a caseload and deliver care in a cost-effective manner that demonstrates an understanding of hospice reimbursement methodologies under the direction of an RN.

19.  Participate in evening/weekend call as required, conducting on-call services in a clinically competent and responsive manner.

20.  Maintain clinical records, statistics, reports and records for reporting and evaluating purposes in compliance with local, state and federal laws.

21.  Participate in staff meetings, department meetings, team meetings, briefings, in-services, committees and other related activities as needed.

22.  Assure compliance with local, state and federal laws, Medicare regulations and established company policies and procedures.

23.  Meet or exceed delivery of company service standards in a consistent manner.

24.  Conduct all business activities in a professional and ethical manner.

25. Maintain all appropriate state nursing licensing requirements.

 

JOB SUMMARY

 

Provide skilled professional nursing care to patient/family/caregiver as directed by the supervising Registered Nurse or the Director of Clinical Services, in compliance with all applicable laws, regulations, and company policies and procedures.  Provide timely documentation of patient services based upon the plan of care.  Identify other physical, psychological, social and spiritual needs for assigned patients.  Work closely with other members of the interdisciplinary team to ensure all patient and family care needs are being met.  Manage related expenditures in a fiscally responsible manner in accordance with the company budget, travel as necessary.

WORKING CONDITIONS

 

General office, extended care facility, patient home setting, exposure to infectious diseases, travel by automobile and occasionally by airplane.

 

 

PHYSICAL DEMANDS

 

Requires moderately heavy physical exertion on a regular and recurring basis such as extensive driving, assisting patients in transfer activities and providing substantial support to individuals in ambulation therapy.  While performing the duties of this job, the employee is regularly required to sit, stand, walk, reach with hands and arms, twist and occasionally stoop, kneel, crouch or crawl.  Regularly required to use hands to fingers, to handle or feel, and talk or hear.  Specific vision abilities include close, peripheral, depth perception, and ability to adjust focus.

 

 

Requirements:

1.  LPN licensed in the state.

2.  Graduate of accredited practical nurse program.

3.  Minimum of one year of nursing experience.  Hospice, home health, skilled nursing facility or medical/surgical experience preferred.

4.  Understanding of hospice philosophy and needs of the terminally ill, thorough knowledge of hospice rules and regulations.

5.  Compliance with accepted professional standards and practices.

6.  Physical exam in accordance with state law requirements.

7.  Ability to work in an interdisciplinary setting.

8.  Ability to submit high-quality written documentation in a timely manner.

9.  Computer skills and experience with Word and Excel.

10.  Organized work habits, proven attention to detail, and ability to multi-task.

11. Demonstrated history of review and improvement of quality assurance or related field.

12.  Satisfactory references from employers and/or professional peers.

13.   Satisfactory criminal background check.

14.  Presentation of negative TB skin test and/or CXR and other tests as required by hospice policy and/or state law.

15.  Valid driver’s license and an automobile that is insured is accordance with state law and is in good working order.

16.  Demonstrate excellent observation, good nursing judgment and communication skills. 

17.  Self-directed with the ability to work with little supervision.

18.  Flexible and cooperative in fulfilling all obligations.

 

 

CONTINUING EDUCATION REQUIREMENTS

 

Employee is expected to participate in appropriate continuing education as may be requested and/or required by their immediate supervisor.  In addition, employee is expected to accept personal responsibility for other educational activities to enhance job-related skills and abilities.  Employee must attend mandatory education programs.
